WHAT IS ASSERTIVE COMMUNITY TREATMENT?

Assertive Community Treatment, or ACT, is a program designed to deliver a full range of services to individuals who have been diagnosed with severe disabling mental illnesses.

ACT’s goal is to provide care within the community to empower consumers to lead a life that is not dominated by mental illness.

How does ACT Work?

ACT consists of a team of Psychiatrists, Nurses, Therapists and other mental health providers that work together to enhance the mental and physical well being of consumers

works to provide services in the community and home where support is needed  most

provides a broad range of services and supports to as it is needed

provides individualized care to each consumer based on personal needs and their  level of functioning

services are available 24 hours a day, 7 days a week

  • Substance abuse treatment

  • Intensive case management

  • Skills training for activities of daily living

  • Assist in the development of social support systems
