ACT: Assertive Community Treatment

WHAT IS ASSERTIVE COMMUNITY TREATMENT?

Assertive Community Treatment, or ACT, is a program designed to deliver a full range of services to individuals who have been diagnosed with severe disabling mental illnesses.

 

ACT’s goal is to provide care within the community to empower consumers to lead a life that is not dominated by mental illness.

How does ACT Work?

ACT consists of a team of Psychiatrists, Nurses, Therapists and other mental health providers that work together to enhance the mental and physical well being of consumers

Act Provides

  • services in the community and home where support is needed  most
  • a broad range of services and supports to as it is needed
  • individualized care to each consumer based on personal needs and their  level of functioning
  • services available 24 hours a day, 7 days a week

What services does ACT PROVIDE?

  • Comprehensive assessments and follow–ups
  • Psychiatric care & coordination of other health services
  • Substance abuse treatment
  • Mental health treatment
  • Intensive case management
  • Medication monitoring
  • Skills training for activities of daily living
  • Coordination of community resources
  • Assist in the development of social support systems
  • ACT 24 hour crisis hotline
