If you need help managing your physical and mental health, our patient navigators
can assist you. We work with people ages 5 and up to support navigation of the
healthcare system and connect you with community resources to meet your unique needs.
What we can do for you:
- Provide personalized assistance from a trained care coordinator
- Serve as your main point of contact for information about services you may qualify for
- Help you keep up with your medications and schedule appointments
- Connect you with medical and mental health providers, specialists, dentists and more
- Provide phone check-ins and in person visits with a trained care coordinator
- Assist with managing complex medical, mental health, and social needs
- Work with you to create a care plan that can support your health and wellness goals
- Connect you with housing, food, transportation, and other community resources
- Coordination of care between your doctors, therapists, pharmacy, hospitals, and more
